Question: In elderly patients with advanced dementia (MMSE of <10 or who are dependant on all care needs) who are currently taking cholinesterase inhibitors or memantine, what is the evidence that the discontinuation of their medication versus continuation of their medication results in furthering worsening of either their cognitive (as measured in MMSE) or functional state (as measured by BADLs), or increased mortality rates?


Overall, there is insufficient evidence available to make any clinical recommendations, and it appears that more research is needed in this area. The authors of one included study did not state any specific clinical or research implications.
